CONFERENCE REGISTRATION



If your institution will pay by check, your invoice will be the confirmation page at the end of the registration process. Please be sure the invoice is included with payment.

If you are paying by credit card, please complete the registration form to create your invoice.

REFUND POLICY

· Anyone requesting a refund 30 or more days prior to conference can receive a full refund of registration fees.

· Requests received between 15-30 days will receive 80% of the total paid.

· Less than 15 days must be authorized by the conference chair and substantiated as an emergency. Subject to $100 cancellation fee if granted.
