Description: Receiving feedback should result in change and the same is true with a feedback form itself. After presenting on "Feedback Form 3.0" at ICLCA 2023, I took the feedback from the audience and started thinking about Feedback Form 4.0. Keeping the dynamic piece to the survey but removing all the clicks, I think we may have found the sweet spot between a survey that gathers useful information but is also quick to complete. Come learn about Feedback Form 4.0 and, maybe, help create Feedback Form 5.0.
